Qualifications:
- Education: Master's degree in Nursing from an accredited program.
- Licensure: Must have an active New Jersey State Nurse Practitioner license.
- Experience: New graduates are welcome to apply.
- Technical Skills: Proficient in performing comprehensive geriatric assessments, medication management, wound care, chronic disease management, and utilizing electronic health records.
- Soft Skills: Strong clinical judgment, excellent communication and interpersonal skills, empathy, patience, and the ability to work collaboratively with nursing home staff, families, and physicians.
Key Responsibilities:
- Provide comprehensive primary care services to nursing home residents, including physical examinations, health assessments, and preventive care.
- Diagnose and manage acute and chronic medical conditions commonly seen in the elderly, such as hypertension, diabetes, heart failure, dementia, infections, and respiratory illnesses.
- Prescribe medications, monitor for adverse drug reactions and polypharmacy, and adjust treatment plans as needed.
- Order, interpret, and analyze diagnostic tests and laboratory results.
- Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team (nurses, social workers, rehabilitation therapists, and attending physicians).
- Monitor residents' health status through regular rounds, assessments, and communication with nursing staff.
- Educate residents and families on disease management, medication adherence, treatment options, and end-of-life care when appropriate.
- Respond to acute changes in resident condition and coordinate timely interventions or transfers to higher levels of care.
- Maintain accurate, thorough, and timely documentation in electronic medical records in accordance with nursing home regulations.
- Participate in quality improvement initiatives, care conferences, and family meetings to optimize resident outcomes.
